Is this insurance?
No. It’s a discount plan, not insurance. No claims, no deductibles, no annual maximums. You pay us directly for member pricing.
Who is the membership plan for?
Patients without dental insurance, patients whose coverage is limited or maxed out, and anyone who wants predictable pricing without insurance paperwork.
How much does it cost?
$299 a year for a single member, $575 for two, $752 for a family of three, $917 for a family of four, and $110 for each additional household member. Paid once a year at enrollment.
What’s included?
Your comprehensive exam and annual exams, unlimited problem-focused exams, consultations, all diagnostic x-rays and 3D CT scans, two cleanings, fluoride, and oral cancer screenings, at no charge. Everything else is 30% off: fillings, crowns, root canals, implants, dentures, whitening and more. Invisalign is $1,000 off full treatment or $500 off Express.
What if it’s been years since my last visit?
Then it’s for you. There are no health questions and nothing to qualify for. Your first exam is included.
How do I join?
Call or text either office, or ask at the front desk at your next visit. Enrollment takes about five minutes and your benefits start the same day.
Can I use the membership at both locations?
Yes. Your membership is valid at both our Cypress (Fry Road) and Katy (Westheimer Parkway) offices.
Does it renew automatically?
Yes. Your membership renews each year on your enrollment date using the card on file, so your benefits never lapse. You can cancel any time before your renewal date by calling either office.
Is there a contract?
No long-term contract. Membership runs one year from your enrollment date and renews automatically unless you cancel before your renewal date. Plan fees are non-refundable.
Can I combine it with insurance?
No. The plan cannot be combined with dental insurance or another discount plan.
What if my plan lapses?
There is a $50 reinstatement fee to reactivate.
Plan terms
- A discount membership plan from Smile Avenue Family Dentistry. You pay us directly for member pricing. This is not dental insurance. Valid at our Cypress and Katy offices only.
- The annual fee is due at enrollment and is non-refundable. Your membership renews automatically each year on your enrollment date using the card on file; you can cancel any time before your renewal date by calling either office. There is no physical membership card to carry. A $50 reinstatement fee applies if your plan lapses.
- Your portion of any bill is due the day of service. Unpaid services are billed at our usual and customary fees.
- Cannot be combined with dental insurance or another discount plan.
- Family plans cover one household. Dual is a spouse/partner or a parent and child. Children are covered under 18, or to 23 if enrolled full-time in college; a child who ages out stays covered until the membership year ends.
- Invisalign pricing ($1,000 off full treatment, $500 off Express) is the member price and is not combined with the 30% discount. It requires an active membership for the length of treatment. Members are never singled out for rate increases or cancellation.
